Memo to pediatricians: Screen all kids for vitamin D deficiency, test those at high risk

ScienceDaily (Feb. 22, 2012) ? As study after study shows the fundamental role vitamin D plays in disease and health, vitamin D deficiency — which often develops insidiously in childhood — should be on every parent’s and pediatrician’s radar, say physicians from the Johns Hopkins Children’s Center.

“Vitamin D deficiency can be a problem year round, but because sun exposure is critical for vitamin D synthesis and production, the winter months further exacerbate what is a perennial problem,” says Johns Hopkins Children’s Center endocrinologist Dominique Long, M.D.

Levels at or below 20 nanograms per milliliter are considered suboptimal. Levels below 15 constitute deficiency and should be treated with supplements.

Hopkins experts say pediatricians should screen all children for risk factors and order blood tests for those found to be at high risk. Children at risk for vitamin D deficiency include:

  • those with vitamin D-poor diets
  • breast-fed infants because breast milk contains minimal vitamin D
  • obese children
  • those with darker skin because darker skin synthesizes less vitamin D from sun exposure than lighter skin
  • those with certain medical conditions, including cystic fibrosis, type 1 and type 2 diabetes and certain gastrointestinal disorders, such as inflammatory bowel disease, which can interfere with food absorption
  • Several large-scale studies have found that vitamin D deficiency is widespread — one in 10 U.S. children are estimated to be deficient — and that 60 percent of children may have suboptimal levels of vitamin D.

Prolonged and untreated vitamin D deficiency can affect multiple organs and functions, including bone growth and density, metabolism, heart and immunity, but it rarely causes overt symptoms and often goes unnoticed.

Vitamin D deficiency in childhood can cause skeletal deformities, brittle bones, frequent fractures and lead to premature osteoporosis in later life. However emerging evidence suggests that vitamin D is involved in far more than bone health. Recent studies have found a link between low vitamin D levels and some cancers, heart disease, suppressed immunity and even premature death. These studies do not show that vitamin D deficiency can cause cancer or heart disease, experts caution, but do suggest that vitamin D may be a powerful player in the genesis of such disorders.

Much of our life-long health is pre-programmed in childhood, and many adult diseases are rooted in exposures, lifestyle and diet during the first decade of life, experts say, and vitamin D, or lack of it, is a classic example.

Long says that she sees at least one toddler with rickets-induced bowing of the legs in her clinic every month and at least one patient per year with seizures stemming from low calcium levels. Without sufficient vitamin D, only 15 percent of the dietary calcium is absorbed, and low calcium can, in rare cases, cause seizures and heart-rhythm anomalies, Long says. Other symptoms of low calcium include poor muscle tone, insufficient dental enamel and muscle spasms.

The good news is that once detected, vitamin D deficiency can be usually corrected easily with high-dose supplementation, Long says. To prevent vitamin D deficiency, the American Academy of Pediatrics recommends that all breastfed infants receive supplemental 400 IU daily until they are weaned and start consuming vitamin D-fortified formula or other foods. The recommended daily dietary intake of vitamin D is 400 IU for children younger than 1 year, and 600 IU for those older than 1 year.

In addition, Long says, parents should ensure children get enough vitamin D in their diets. Foods rich on vitamin D include fish (sardines, salmon tuna), egg yolks, vitamin D-fortified milk, vitamin D-fortified orange juice, cereals, yogurt and cheese.

While traditional theories hold that the pyramid was built via a long external ramp, such a ramp would have had to wind around for more than a mile to be shallow enough to drag stones up, and it would have had a stone volume twice that of the pyramid itself.

A new, more economical theory gaining traction among architects and Egyptologists holds that the bottom third of the pyramid’s height was constructed by stones dragged up an external ramp. But above that ? for the remaining 33 percent or so of the pyramidal volume ? the Egyptians worked their way up through the inside of the structure, building around a gently sloping internal ramp and fitting stone blocks into place as they ascended.

Furthermore, the workers could have reused the stones quarried for the external ramp to build the pyramid’s upper echelons, so that nothing went to waste.

Jean-Pierre Houdin, the French architect who developed the internal ramp theory, has collaborated with a team at Dassault Systems, a 3-D graphics firm, to create a virtual model of the construction process. A team of scholars at Laval University in Quebec is now planning an infrared imaging investigation, which could soon reveal the spiraling ramp within the Great Pyramid; if found, it will be the final proof of Houdin’s theory. But whether or not the theory bears out, Houdin says an inside-out construction would still be the best way to build the Great Pyramid.

“I am quite sure we could do the same today, and it would be the most economical method,” Houdin told Life’s Little Mysteries.

There would be two main differences between pyramid-building now and then. First, “Instead of people pulling the sleds that carry the stones up the ramps, you would use something with an engine,” he said. Secondly, “for the (topmost) 10 or 15 meters, you would use a small crane.”

Just as cranes are lifted onto the tops of skyscrapers today, a helicopter would apposition a crane onto a flat top of the pyramid. Stones and other construction materials dragged up to that level via the internal ramp would then be set in place by the crane. (It wouldn’t be feasible to build the entire structure with cranes, Houdin said, because they wouldn’t be able to reach far enough to lift materials from the base to the center of the top of the pyramid.)

While the pyramid was originally built by 4,000 workers over the course of 20 years using strength, sleds and ropes, building the pyramid today using stone-carrying vehicles, cranes and helicopters would probably take 1,500 to 2,000 workers around five years, and it would cost on the order of $5 billion, Houdin said, based on manpower and cost of constructing the Hoover Dam in Colorado during the Great Depression. The dam contains a volume of concrete roughly equal to the stone in the pyramid. By comparison, the 1,776-foot-tall One World Trade Center being constructed in downtown Manhattan will cost an estimated $4 billion.

There are no plans to build a full-scale Great Pyramid, but a campaign for a scaled-down model is under way. The Earth Pyramid Project, based in the United Kingdom, is raising funds to erect a pyramidal structure in an as-yet-undecided location, built of stones quarried all around the world. It will contain a time capsule, to be opened 1,000 years from now.

Funded by governments and organizations around the globe, the Earth Pyramid will not only provide a window into contemporary culture for future societies, it will also serve as an opportunity to test Houdin’s construction theory of the Great Pyramid of Giza.

Benefits of Religious Faith:? All in Your Head?

It?s an important question, especially as more and more people jettison religious faith in favor of more subjective, personal, spiritualities.? According to the Pew Research Center, the rate of 18-29 year olds jettisoning religion has doubled from 8% to 16% over the last 10 years.

If religion is merely a personal affectation?something done in private for one?s own pleasure?then throwing it over for a more self-styled spirituality?or nothing at all?just makes sense.? But what if religion?offers more specific and robust?benefits to?psychological health and the health of society than personal spirituality?? What if there?are objective?benefits to following a specific creed, participating in specific rituals, and actively associating and worshipping with a particular group on a regular basis?

Does the person and society as a whole lose something if it jettisons, specifically, religious faith?

A Question of Science, Not Faith.

I want to be clear that these questions go beyond both?the truth claims of any particular religion and the issue of whether religion is personally meaningful or not.? The question of?any objective benefit?of?religion to the person?or society?is really one of social science?psychology, anthropology, and sociology, especially.? Obviously, religious people subjectively?think that religion is useful, but why? Perhaps they?re just deluded.? Psychologists going all the way back to Freud have argued that possibility.? Does it really all just come down to religion giving people a warm-fuzzy feeling inside?? ?And why should non-religious people care about religion at all?

Freedom and Dignity

It would be easy to say that religion concerns itself with ?the big questions.?? But the truth is, no one needs religion to (with apologies to Douglas Adams) contemplate life, the universe, and everything.? Almost everyone is fully capable of asking those questions without a creed, rituals, or?a group to support them.

But I would argue that what a person and society does need religious faith for is to proclaim and protect the dignity of the human person and the value of authentic freedom.??Atheists like Christopher Hitchens and Richard Dawkins have correctly noted that you don?t really need religion to be?moral or good.???? But?even a casual look at the data seems to suggest that?as religiosity of a particular society decreases, human rights abuses increase.

As a society actively (like in communist countries) or passively (as in the consumerist West) stamps out religion (and?I?m not just speaking of Judeo-Christian faiths, but?virtually all religious faiths)?two things happen:??First, society begins to see people as things.? Second, people begin to see themselves as things.? And when people become depersonalized, ?thing-ified? if you will, psychosocial health suffers to the point of non-existence.

Reason?Without Faith is Nihilism

People are capable of great feats of reason on their own, but without religion, people struggle with significance.? There?s an old saying that goes, ?Some people know the price of everything but the value of nothing.?? That?s reason without faith.?? Without faith, people too easily begin to think of themselves and each other as things, as merely means to a particular end.? Every person has a price.? But no one is valued.

Religion:??Meet Needs AND Respect the Person?

Human beings tend to be pragmatic by nature and we stink at delaying gratification.? We see a need and we want to fill that need in the quickest way possible.? Unfortunately, the quickest way to fill a need usually involves treating people (others and ourselves) as things.? When?I?m hurting, the last thing I?m prone to care about is human dignity and?freedom.??I just want to meet my need.? Now.? By any means possible.??What?s the quickest way to meet a sexual need?? Porn or prostitution.? What?s the quickest way to win a domestic argument??? Treat my spouse like a punching bag.? What?s the quickest way to meet an economic need?? Treat workers like slaves.?? What?s the quickest way to achieve a political goal?? Treat people like?cattle to be hearded?instead of citizens?deserving of?respect.

And though these may be the quickest means to meet particular ends, they are hardly means that respect the dignity and worth of the human person.? The thing is, it is difficult to object to these practices on reason alone.? All reason can tell us is that people are bags of meat.?Reason can tell us that consciousness exists, but it?can?t tell?us the significance of that fact.? Reason can tell us that, generally speaking, it?s?better for me to treat others with respect?unless I can get away with doing otherwise.???If I have the power to shield myself from the consequences of my actions, why?shouldn?t I treat people as things if it would benefit me?? It simply isn?t reasonable to think otherwise.

Likewise, because they are personal, individual spiritualites tend not to stand up well to social pressure.? Individual spiritualities thrive where religions have made it safe for them to grow.? But when religous faith is stamped out, mere pragmatism?the organized thing-ification of persons?kills spirituality altogether.

The more a person is treated, or treats themselves as a thing, the more they break down.? The cost of porn?? Sexual addiction, depression, lost work hours, broken marriages and families.? The cost of prostitution and sexual trafficking?? Post-traumatic stress, depression, addiction.? The cost of domestic violence?? Depression, suicide, anxiety, addiction, broken marriages and families, and an increase in?all manner of psychosocial disorders in?children.?? The cost of inhumane working conditions?? Depression, suicide, anxiety.? The cost of political oppression?? All of the above.

The Life You Save May Be Your Own

When mainstream?religious faith is in conflict with a particular society as a whole, or with particular groups within a society, it is almost always because?a religious faith is asserting that?a?particular means by which a person or society is?attempting to?satisfy a particular need?runs contrary to the?dignity?or freedom of the human person.

Whatever you think of the nature of their respective creeds, Christianity, Buddhism, Falun Gang, are all mercilessly persecuted in China because they assert that persons must be more than cogs in the system.? Whatever you think of their particular claims,?progressive Christianity came under attack during the civil rights movement just as ?traditional Christianity is?now under legislative attack?in the US because it asserts that certain efficient means of solving social problems are beneath the dignity of the human person.? In every case, it is organized, religious faith?much more than even individualized spiritualities which, because they are personal, tend to be easily subverted by outside pressures?that has the power to inspire people to realize that they are worth more than others (and even they, themselves) say they are.? It is religious faith that consistently asserts, as?point of revelation as?opposed to mere reason, ?that people must be treated as persons, not things.

Moreover, it is organized?religious faiths, more than personal spiritualities, that have the organizational?power to demand social change and?encourage?personal growth and change in a manner that is respectful of the person and the groups those persons participate in (family, work, society).

So, whether you are a believer or not,?the next time?religious faith stands in opposition to a pet solution you favor, stop and consider whether your impulse is driven by a pragmatism that has not considered the dignity and worth of the people around you.????As Flannery O?Connor once put it, the life you save may be your own.

King Cake bars for Mardi Gras

I have never set myself the task of making a real King Cake, as I figure there are enough people who do that already. My brother used to send me delicious King Cakes from a bakery in New Orleans when I was in college. Those cakes were moist and tender with a cream cheese filling, with a brightly colored, but simple frosting.?

The cake was packaged with beads and doubloons and made for a fun party all around. I was always very popular during Mardi Gras season. That is really my idea of what a King Cake should be, though I know there are many different versions. Those were the first, and still the best, King Cakes I have ever had. But now, I often find the bakeries in my local grocery stores offer wildly decorated King Cakes leading up to Mardi Gras. I have bought them, but they are generally dry and tasteless and a real disappointment (and often stale). I once ordered a fancy, artfully decorated King Cake from a New Orleans bakery at great expense, but even it was dry.

These bars meet all my King Cake flavor requirements, but are simpler to make and to serve. They make a great dessert for a Mardi Gras party, and would pack up beautifully to carry to a parade-viewing spot. I sprinkle colored sanding sugar in the traditional purple, green and gold color scheme over the top for a sparkly Mardi Gras feel, but you could easily tint the glaze, use sprinkles or the fancier luster dust.

And a word about the baby. Traditionally, King Cake has a small plastic baby figure backed inside. The person whose piece of cake contains the baby is then responsible for hosting the next King Cake party. Many bakeries now include the baby in the box, but don?t bake it into the cake. I assume this is for liability reasons, as a small plastic baby is a choking hazard. If you do happen to have a plastic baby, feel free to bake it into the King Cake Bars, though it is likely to show through the filling and be less of a surprise.

King Cake Bars
Makes about 15 bars

For the Crust:

2 cups all-purpose flour
1 cup (2 sticks) cold butter
6 tablespoons sugar
2 tablespoons milk

For the Filling:

2 (8-ounce) blocks cream cheese, softened
1 cup sugar
1 egg
1 teaspoon vanilla extract
1/2 teaspoon cinnamon

For the Glaze:

1 cup confectioners? sugar
2 tablespoon milk
Colored sanding sugar or sprinkles (purple, green and gold)

For the Crust:

Preheat the oven to 350 degrees F.? Spray a 9- x 13-inch glass baking dish with cooking spray.

As a woman who has had lumpectomy on both breasts, I thank my lucky stars for that simple yet effective operation. But a new study published recently in the Journal of the American Medical Association brings up a worrisome issue: medical guidelines dictate when lumpectomy should be an option, but don?t describe how much tissue beyond the tumor should be removed.

Surgeons remove a segment of tissue around the tumor in an effort to get ?clear margins,? without any cancer cells. Without medical guidelines dictating how much is too little and how much is too much, surgeons decide on the amount of tissue to be removed based on their own insight and intuition. This study showed, however, that in some cases, those qualities fall short and the patient pays the price.

This research shows that some women who need more surgery aren?t getting it, while others are having unneeded surgeries. . . . It is an indication of the need for some level of surgical guidelines regarding lumpectomy.

Reviewing the records of 2,206 lumpectomy patients at four hospitals in different areas of the country, researchers found that about 23 percent of the women had a repeat lumpectomy, even though about half of them had clear pathology reports. What patient benefit did those second surgeries serve? Even more troubling, 14 percent of the women studied were found to have residual breast cancer cells after lumpectomy, yet they did not return to the OR?the reasons for this weren?t identified. The rates of repeat surgery also varied wildly among surgeons, ranging from zero to more than 70 percent. What would validate one surgeon having do-overs on almost three-quarters of his or her patients?

Every breast cancer case is different, so it?s understandable that lumpectomies might not be performed exactly the same way for all women. But the study results underscore one fact: presented with the same lumpectomy, surgeons have differing opinions of how much tissue should be removed ?to be safe.? Some like to err on the side of caution, removing a wide margin of tissue, while others take a more conservative approach, trying to remove cancerous tissue while preserving the shape and volume of the breast as much as possible. Most importantly, this research shows that some women who need more surgery aren?t getting it, while others are having unneeded surgeries.

It?s not clear whether the findings of this one study would hold up if all lumpectomies were subjected to the same scrutiny, but it is an indication of the need for some level of surgical guidelines regarding lumpectomy. Hopefully this will motivate surgeons and the appropriate health officials to do what is needed?recognize that each patient case is different, but establish guidelines that help surgeons become more effective and ensure that fewer women experience the frustration and anguish of a repeat lumpectomy.

In the meantime, what should women do if they?re facing a lumpectomy? Be your own advocate: ask questions, research your options, and get a second opinion before any surgery. Insurance usually pays for getting a second opinion; it takes just a bit of time and will be well worth the effort.
